**Remainder of Calendar Year Deductible: City pays remaining deductible at 100% from $376 to $ 2700 for each covered person up to the family maximum.
**Co-Insurance: Health Insurance Company pays 80% of the next $8,500 of eligible claims. The employee pays the 20% of the first $3,000. Then the City pays the other 20% of the next $5,500. The Health Insurance Company then pays 100% thereafter
